Center for Integrative Psychotherapy is now offering telehealth services through Hipaa and HI-TECH Compliant Zoom Healthcare application.
If interested, please do the following:
1. Please call your insurance company to verify that you are covered for telehealth services under your plan, please make sure to specify your provider's name and credentials when calling, and please obtain and save a call reference number
2. Go to our forms tab and download the Intake forms
3. Complete the forms and either scan them or take a picture of them, and send them to cip.admin@cip-cbt.com or fax them to 610-432-0973. Your telehealth appointment will not be confirmed until we receive all forms completely filled and signed
4. Call our office at 610-432-5066 to schedule an appointment and to provide us with an email address or phone number, so that you can receive the invitation to your telehealth meeting through Zoom App.
5. Download the zoom app. (Available at your phone app store or online)
6. You will get an email with an invitation to access the meeting. Email contains meeting number and password. That is all you need to access the meeting.
7. Before your appointment please give us a call to pay for your co-payment or deductible cost share responsibility.
